Using the MAF shown in this picture:

I assume that this is what a MAF on a TR is supposed to look like, right? The MAF on my 87 looks like an "hourglass". It gets skinnier in the moddle. What's up with this, is this restricting airflow? What can I use to improve my MAF?

Yes, that is what a stock MAF is supposed to loook like. If you don't have that one, and your's necks down with the "hourglass" shape - IT IS JUNK! Yes, is restricting airflow! Yes, it is robbing your car of power. If you looked at this with a scan tool, your MAF values would be sub par at WOT. Just like your horsepower numbers are. I guarantee it.

AC does not make the stock MAF anymore. So, your choices are (1) a decent rebuilt unit or (2) a LT1 or LS1 MAF and a MAF Translator. Option (1) is pretty difficult to find. Option (2) is not cheap, but is available and is proven to work. And it actually flows better than the stock MAF to boot. One good source for the MAF Translator is http://www.fullthrottlespeed.com/

As you can see, an aftermarket MAF is a BAD idea,

What about the Impala SS MAF? What about the "non necked down style" aftermarket MAF available through partsmamerica.com? Are either of these other available options?

Impala SS = LT1. Requires the MAF Translator. This would actually be an airflow upgrade. And cost some cash to do the complete upgrade. But it would be worth it.

Units from Parts America might work OK. Might not. Their catalog shows 'new' Borg Warner and Niehoff units that look OEM. As well as a Niehoff reman that appears to be an OEM reman. They also show a aftermarket Cardone unit that is still full size, not necked down. Also a Python aftermarket unit with no picture; beware. Caveat emptor.

Choose wisely.

What is the extender T? How much did this setup run you and where did you get everything?
